Graduation Request Form

Use this form if you would like to receive your official UBC Certificate and final transcript. Ensure you have completed all course requirements per the program curriculum at the time you enrolled in the program before submitting a graduation request.

Please note: There is a $50 fee for Paper Certificates, which may take up to 3 months to receive. There is no fee for a Digital Certificate, which will take between 1-2 weeks to receive.
